  3. A new page will load where you can then either: A) Pay be credit card by clicking on the "Pay With Your Credit Card" button, or B) log into your PayPal account to pay the amount.


Important!: If you plan to seek reimbursement from your extended health insurance company, your online receipt verifies your payment, but otherwise provides insufficient information for your insurance company to process the claim. When you submit your claim, therefore, along with your receipt you will need to include the invoice you received from Cobb & Associates Inc. which verifies the provider's information, the date of service, the specific type of service, and the person who received the service.
